i had to go to the ER yesterday and they gave me a rectal exam. i guess through all the commotion i didn’t think to ask "how come every time i push i get some relief, am i getting this bulge next to my rectal area" and he didn’t say anything so is this bulge a hemorrhoid or is it just swelling or what? wouldn’t the doctor have noticed if something was wrong? please help me out and let me know if taking otc medications and other techniques is a good idea just to be safe. please answer thank you

The bulge is likely the hemorrhoid. If there is anything more serious involved the doctor would not know unless it is in the anal/rectal area. You should have been given instructions and told to follow up with your own doctor. ER docs stabilize patients and send them home or have them admitted, that is their job they only deal with emergent situations. If you want a full evaluation you need to go to your doctor.
