I think the condition is actually called strangulated hemorrhoid, and as far as I understand it, the strangulated hemorrhoid is a prolapsed hemorrhoid (an internal hemorrhoid which has pushed through the anal opening) who has the blood supply cut off by the anal sphincter.

A thrombosed hemorrhoid is an external hemorrhoid, so it develops outside the lower rectum from the beginning, and it's defined by a hard lump or swelling formed by a blood clot.

The condition you are describing points indeed to the conclusion that your hemorrhoid ruptured. This is quite common and it happens when the blood pressure in the veins increases, which leads to stress and ruptures. The high blood pressure was most probably caused by a sudden movement, stretchering, increased heart beat or something similar.

Hope this helps :)

As far as I'm aware this is the main difference. When veins in the anus or outside the rectum star to swell and external when they protrude outward from these areas. When blood flow becomes restricted in these veins, the vessels tend to split, causing pools of blood to form and clot under the skin, when this happens you get a thrombosed hemorroid.

Whereas a strangled hemorroid is a prolapsed hemorroid that has become trapped by the anal sphincter, causing the blood supply to become occluded by the sphincter's constricting action.
